Free and Low-Cost Addiction Treatment in NH and MA: Who Qualifies

If you are searching for free rehab in New Hampshire or Massachusetts, the most useful thing to know is that "free" usually means "paid for by someone else," and in these two states that someone is most often Medicaid. New Hampshire's Medicaid program covers substance use treatment for adults with low incomes, and so does MassHealth in Massachusetts. For people who do not qualify, both states run free phone lines that connect anyone to treatment, and federal rules put pregnant women and people who inject drugs at the front of the line at state-funded programs.

This guide covers who qualifies, how to apply and where to call first in each state. It is general information drawn from the state and federal sources listed at the end, not legal or benefits advice. Who qualifies for free rehab in New Hampshire?

NH Medicaid and the Granite Advantage Health Care Program

For most adults in New Hampshire who cannot pay for treatment, the answer is NH Medicaid. According to NH DHHS, the Granite Advantage Health Care Program covers people who:

• are at least 19 and younger than 65
• are not pregnant when they apply (pregnant women have their own Medicaid coverage)
• are not entitled to or enrolled in Medicare Part A or B
• do not qualify for another mandatory Medicaid group
• have household income at or below 133 percent of the federal poverty level

You also need to live in New Hampshire, be a U.S. citizen or qualified non-citizen, and provide a Social Security number or proof you applied for one. Income is counted using federal tax rules, so your tax filing status and dependents decide your household size.

Most NH Medicaid members get their benefits through one of three health plans: AmeriHealth Caritas New Hampshire, NH Healthy Families and WellSense Health Plan. DHHS notes the plans cover the same Medicaid services but can have different provider networks and rules, such as prior authorization, so it is worth checking that a program is in your plan's network. You can apply through NH EASY or call the DHHS Customer Service Center at 1-844-ASK-DHHS (1-844-275-3447).

One change to plan for: NH DHHS says that beginning in January 2027, people covered through Granite Advantage will face work or other community engagement requirements, more frequent renewals and limits on retroactive coverage, following the federal law passed in 2025. If you have Granite Advantage, keep your address and phone number current with DHHS and open every letter.

The Doorway, by dialing 211

If you are not sure where to start, dial 211. It connects you to The Doorway, New Hampshire's entry point for substance use help, based at nine community hospitals, including The Doorway of Greater Manchester at 300 Hanover Street and The Doorway of Greater Nashua at 268 Main Street. The Doorway offers screening and evaluation, connection to treatment including medication-assisted treatment, naloxone and recovery support, and it helps family members who are calling about someone else.

NH DHHS says many treatment services in the state are paid for by public or private insurance, with funding from DHHS to help people get into treatment. The Doorway and 211 are how you reach those state-funded options.

Who qualifies for free rehab in Massachusetts?

MassHealth

MassHealth, the state's Medicaid program, covers substance use treatment through its plans. According to MassHealth, eligibility depends on:

• your income, measured as Modified Adjusted Gross Income under federal tax rules
• who is in your household
• living in Massachusetts
• your citizenship or immigration status, verified by the state

MassHealth offers different coverage types for parents and caretakers, young adults, adults 21 through 64, pregnant people, people with disabilities and others, and the only way to know which one fits is to apply. One application covers MassHealth, the Health Safety Net and the Children's Medical Security Plan, for anyone under 65 who needs coverage and help paying for it. You can apply online with an MA Login account, or by phone, mail, fax or in person.

Free helplines in Massachusetts

The Department of Public Health lists free and confidential helplines that connect anyone to treatment:

• Massachusetts Substance Use Helpline: call or text (800) 327-5050, or chat at HelplineMA.org. Free, 24/7, 365 days a year, with a warm handoff to programs when needed.
• Behavioral Health Help Line: call or text (833) 773-2445. Free, no insurance needed, for mental health and substance use, with clinical assessment and treatment referrals.
• Peer Recovery Support Centers: free, peer-run spaces for people in recovery and their families.

Who gets priority at state-funded programs?

Programs that receive federal substance use block grant money have to follow federal priority rules. Under 45 CFR 96.131, each pregnant woman who seeks or is referred for treatment is given preference in admission. Under 45 CFR 96.126, a person who needs treatment for injection drug use must be admitted within 14 days of asking, or, if no program has room, within 120 days with interim services available within 48 hours. If either applies to you, say so when you call.

What if I do not qualify for Medicaid?

Start with the free lines, 211 in New Hampshire and (800) 327-5050 in Massachusetts, which know the state-funded options near you. Nationally, the SAMHSA National Helpline at 1-800-662-4357 is free, confidential and open 24/7. 1. Call the free line for your state. In New Hampshire, dial 211 for The Doorway. In Massachusetts, call or text the Substance Use Helpline at (800) 327-5050.
2. Apply for Medicaid if you might qualify. Use NH EASY in New Hampshire, or the MassHealth application in Massachusetts.
3. Have any insurance checked. A benefits check costs nothing and tells you what you would actually pay.
4. Mention priority criteria. Pregnancy and injection drug use move you up at state-funded programs.
5. Ask what support is available while you wait. For pregnant women and people who inject drugs, federal rules require interim services when a state-funded program has no room.
